Understanding Hairline Placement in Hair Transplants

Introduction

Planning the hairline is one of the most important aspects of design. Proper hairline placement requires artistry, imagination, and experience. Many factors must be considered; these include an age of the patient, the amount of future hair loss, the colour of hair, the pattern of remaining hair, desires of the patient, postoperative hairstyles, and the number of anticipated reductions.

Overview

Conservative, high hairlines are favoured by nearly all experienced surgeons for the following reasons:

1. The slightly unnatural graft edge is easier to conceal with any remaining hair if the hairline is placed well back on the forehead.

2. A high hairline can always be brought forward later, but it is difficult to change a hairline that is too low.

3. The grafted hair is usually angled anteriorly so that a full-haired appearance can be created without an actual low hairline.

Most surgeons tend to create the same hairline, varying it only to fit the existing pattern. Design and placement of hairlines become almost a signature for the surgeon and his patients.

Angling and Direction of Grafts

The proper angle and direction of hair growth are almost as important as graft placement. The surgeon should consider at what angle and direction he wants the hair to grow so that it will provide the most apparent density and coverage. The angle and direction must be coordinated with the anticipated hairstyle and should be carried throughout the entire design. Changes in the angle and direction from one area to another should be gradual and flowing. Generally, the anterior and radial direction of the vellus hair is correct, but sometimes increasing the angle or slightly changing the direction is advisable, particularly at the hairline and at the calyx.

Location of Grafts and Probable Hairstyle

The best quality grafts should always be placed where they will afford the most coverage. This is particularly important in marginal candidates or in patients with limited amounts of donor hair. Consideration of the future hairstyle is particularly important in this aspect of the procedure. Grafts should be concentrated and even crowded into the front hairline particularly on the side where the hair is parted. Parts through the grafted hair usually appear unnatural and so should be avoided. Care must be taken to place the grafts so all the hair will comb in the same direction. Occasionally, however, this rule can be broken. In individuals in whom the existing hair is low on the sides of the head, and reductions for one reason or another have not been performed, parting through grafted hair can be done. This should definitely not be the routine, but with carefully selected patients the appearance is satisfactory.

The policy of concentrating the grafts on the sides of the part should be followed even on the vertex and crown. After the crown and vertex have been reduced, there is always a scar, and with central reductions, the hair may tend to radiate away from the scar. The grafts should be placed adjacent to and down the centre of the scar with the hair angled and directed so that it will provide the most coverage and conform most naturally with the proposed direction of hairstyling.

It is not possible to produce a completely natural grafted hairline, although in some cases the appearance may be excellent. The patient must be warned of any anticipated problem and the grafts placed in such a way that the future hair growth may be combed forward and across to conceal the grafted hairline.

This rule can be broken with salt and pepper gray hair and occasionally with blonds. These hairlines often look so naturally they can be fully exposed.

In some patients, because of the pattern and extent of the alopecia, the only available hairstyle will be straight back even after reductions have been performed. In these patients, grafts should be screened extremely carefully before commencing, as the final density will not be great. A few well placed, natural natural-appearing grafts providing even sparse hair go a long way towards enhancing the appearance. On the other hand, poor or improperly placed grafts can have just the opposite effect.

FUE hair transplantation is performed by the micromotor -a specially designed microsurgical device- without making any incisions or sutures. Hair follicles are individually removed from the scalp by using the micropunches with diameters of 0.6-0.9 mm inserted to the tip of the micromotor.

Extraction procedure is performed under the magnifier equipment by using the microsurgical technique only. Extraction is performed within the limits of the scalp at the level of the follicles. Since the extraction is limited to the level of the scalp, the tissues located deeper get no harm.

The most critical point to be taken into consideration in this procedure is giving no harm at all -not only to the hair shafts but- to the totality of the hair follicles with the hair shafts they contain, during the extraction. No hair shaft grows from a damaged hair follicle which is transplanted. If the hair follicle (follicular unit) is damaged, no hair will grow and the result will be unsuccessful. If no hair grows after the hair transplantation operation, one of the main reasons of this is a careless extraction procedure performed by an inexperienced person.

Since no hair follicle forms to replace the damaged hair follicle, a successful hair transplantation operation is possible only if the donor site is used with full efficiency. For the complete collection of each hair follicle individually, extraction should be performed carefully by a plastic surgeon.

The distribution of the completely extracted hair follicles is also significant. The number of the hair follicles should separately be calculated for those follicles that contain one, two, three and four hair shafts. Extraction of the hair follicles that contain three or four hair follicles as much as possible and avoiding those follicles containing only a single hair shaft will lead to a denser hair result. While 4000 hair follicles which contain a single shaft correspond to 4000 hair shafts, 4000 hair follicles which contain three shafts correspond to 12,000 hair shafts.

Extraction should not be considered as a simple technical procedure. The diameter of the micropunch, the speed of the micromotor, the orientation and the depth of the hair follicles, and the distance between the extracted follicles should be evaluated separately for each patient. At this stage, the level of experience and the skills of the plastic surgeon as well as the quality of the technical equipment come into play. In this context, we argue that ‘there are no disorders, there are patients’ and we make a separate evaluation for every single patient.

In addition, the total number of hair shafts targeted for 4000 hair follicles is 10,000 hair shafts at a minimum. The extracted hair follicles are stored inside a special nutritive solution in a cooled ambient in order to keep them alive.

Making the canals for inserting the hair follicles (Graft) during the hair transplantation

The canal to be prepared should be as small as possible but it should still be wide enough to allow for a smooth insertion of the hair follicles. In this way, the nurse who performs the insertion can easily adapt the hair follicles to the canals and the blood circulation in the scalp does not get disrupted. As a result, the rate of hair growth from the transplanted hair follicles approximates a hundred percent.

Definitely no visible scar is left and a natural result is obtained as long as the canals are prepared cautiously. Similarly, if the canals are prepared in conformity with the orientation of hair, the recently growing hair can be combed in the desired direction and it looks natural like the original hair.

It is highly difficult to change the inaccurate hair orientation to the proper orientation even with corrective operations. In addition, incisions wider than adequate lead to noticeable canal scars at the bottom of the hair shafts.

While planning the hairline, the canals should not follow a straight and sharp line; they should rather be prepared a little disorderly as a wavy line which gives a more natural appearance.

If an inaccurate architectural planning is made, the result will not be aesthetical and natural even if hair grows after a successfully performed transplantation. For all these reasons, the hair insertion canals should be prepared by an experienced plastic surgeon who is specialized in the area with comprehensive knowledge.

This comprises the implantation of the hair follicles individually extracted by the FUE method into the hair canals prepared. In this last phase of hair transplant, the hair follicles are inserted at the appropriate depth with the appropriate angle without giving any harm to them. This procedure requires patience and caution. The hair follicles should individually be inserted into every single canal without skipping any of them.
